NHS Health checks

The Omnia Practice is taking part in a disease prevention program called the NHS Health Check.

Heart disease and stroke are the UK’s biggest killers but some can be prevented or delayed through early detection of risk factors and appropriate treatment. We will be offering a free NHS Health Check to all patients on our practice list, who are between the ages of 40 and 74, and who are NOT recorded as having heart or arterial disease, stroke, diabetes or kidney disease, and who are NOT taking statins or blood pressure medicines already.

If you are eligible (see above) we would like to invite you to take advantage of this free NHS service.

Filling in the questionnaire (NHS Health Check Questionnaire) and, ideally, returning it to the surgery before your appointment, or bringing it with you to your appointment, will help us with the process.

If you are eligible and would like to take up this offer of a free health check, please contact the practice to make an appointment for a “NHS Health Check Appointment”, during this appointment we will go through your health history (and the form above), we will also take a blood test to measure cholesterol and diabetic levels.

Please be aware that due to demand, there may be a delay in having the health check after you have requested it. We are doing our best to do them as quickly as possible.
