District Nurses

As a team we manage and deliver nursing care to people aged over sixteen years old and who are unable to attend surgery. We treat patients in their own homes, residential homes and within clinics at the health centre. Clinics include Leg Ulcer Clinic, Healed Leg Ulcer Clinic, Stress and Continence Clinics. Management of care for palliative patients, patients with chronic long term conditions and health promotion and health education are also integral parts of our role.

We work together with the GPs, practice nurses, receptionists, hospitals, social workers, social services, occupational therapists, physiotherapists and podiatrists, private and voluntary organisations to provide an identified package of care for patients on our caseload.

Part of our role involves mentoring and assessing pre registration students and post registration district nurse students.
